Appointment Request

Dazen and Cadieux is open Monday through Thursday with hours that can accommodate most any schedule. We will schedule your appointment as promptly as possible. If you have pain or an emergency situation, every attempt will be made to see you that day.

We try our best to stay on schedule to minimize your waiting. Due to the fact Drs. Dazen & Cadieux provide many types of dental services, various circumstances may lengthen the time allocated for a procedure. Emergency cases can also arise and cause delays. We appreciate your understanding and patience.

New Patient Questionnaire


Our office is committed to helping you maximize your insurance benefits. Because insurance policies vary greatly, we can only estimate your coverage in good faith. But cannot guarantee coverage due to the complexities of insurance contacts. Your estimated patient portion must be paid at the time of service. As a service to our patients, we will be insurance companies for services and allow them 45 days to render payment. After 60 days, you are responsible for the entire balance, paid in full. If you have any questions, our courteous staff is always available to answer them.

Your treatment plan will include a breakdown of all applicable fees, and we will inform you of all costs before treatment is administered. Charges are payable at the time of treatment. For you convenience, we office the following payment options to meet your needs.

In order for you to obtain the best possible dental care for your needs, we offer a variety of payment options. We participate in several dental insurance plans and accept many more. Contact us for specific plan details. We also accept Visa, MasterCard and Discover and have a credit plan available to qualified patients. Be sure to ask about our prepayment and immediate payment policies. We are here to assist you!

Please ask a member our staff and we will review our payment options to accommodate your financial needs. If special arrangements are needed, please talk to our office manager prior to receiving service.

Additionally, we will submit all insurance claims for you. We will fully attempt to help you receive full insurance benefits; however, you are personally responsible for your account and we encourage you to contact us if your policy has not paid within 30 days.


Your scheduled appointment time has been reserved specifically for you. If you need to change your appointment, we request 48 hours advance notice, otherwise a $50 charge will be applied. Exceptions to this can be determined on an individual basis according to circumstances.

Your First Visit

When you arrive for your first visit, please be prepared to complete all insurance and health information forms that will allow us to begin your dental treatment. We will ask you to fill out several forms that will get you acquainted with our office. For your
convenience, you may download these forms and bring them with you to your visit.
